New and returning patients – please bring to EVERY appointment at Starfish Pediatric ENT:

Health Insurance Card/Information
The child and ideally 1 or both parents
A friend or family member to take notes/help with questions
A list of all medications the child is taking
A list of your questions /concerns
A copy of your XRAYS / CT scans (not the reports – get the actual films or CD)
Copies of records from doctors related to your child’s ear, nose and throat problems
Completed FORMS – New patients only.
A photo ID

New patients – We know your time is valuable. Please print out, complete, and sign all 5 BLUE forms below BEFORE arriving for your first appointment to ensure minimal wait.

Returning Patients – Thank you for continuing to see us, we know you want to have an efficient visit that focuses on your concerns. Please print out, complete, and sign the RED form below BEFORE arriving for your appointment to ensure minimal wait
